In consideration of                                                        , my child, myself being permitted to participate in any way as a volunteer during the period of                         to                       with the Sustain Cities (DBA make GVL greener) Garbage Gang Program (the “non-profit”), agree to the following. I understand that my participation and/or my child / ward’s participation is a voluntary activity, and that I am or we are freely donating my or our time and labor. I or we agree to perform my or our assigned tasks in a responsible manner. I also hereby agree to ASSUME THE RISK OF PROPERTY DAMAGE, INJURY, ILLNESS, OR DEATH in any way associated with my participation and/or my child participation in this volunteer activity. I agree to RELEASE, DEFEND, INDEMNIFY, AND HOLD HARMLESS the Non-profit and its Garbage Gang Program, its officials, employees, representatives, volunteers, and agents for any and all rights and claims for damages, including attorney fees, I now, or may hereafter have, whether known or unknown, in law or in equity, and arising from or in any way connected with my participation in volunteer activities. I agree that the terms stated herein shall also serve as a WAIVER OR LIABILITY AND ASSUMPTION OF RISK for my heirs, estate, executor, administrator, assignees, and for all members of my family.

I understand that this activity is (maybe) physically strenuous and I know of no medical reason why I and/or my child should not participate. I further understand and declare that I intend to be financially responsible for any injury or fatal occurrence that may occur to me and/or my child during or as a result of such participation or transportation. Neither I nor my child will carry or possess weapons of any kind while participating in this volunteer program. I and/or my child will abide by the established policies and procedures as set by Greenville County.

PHOTO RELEASE

Furthermore, I give my permission to have photos and/or video recordings taken of me and/or my child for publicity purposes during volunteer activities. I hereby grant and convey unto Sustain Cities all rights, title and interest in any and all photographic images and video or audio recordings made by Sustain Cities during my volunteer activities and/or my child volunteer activities with the Non-profit.

CAUTION

I acknowledge that I have carefully read this WAIVER OF LIABILITY AND ASSUMPTION OF RISK and fully understand that I am waiving any right that I may now or hereafter have to bring a legal action to assert any claim against the non-profit, in connection with my participation and/or my child participation in volunteer activities.
